Advance Directives (pdf)
Every patient has the right to complete a written statement about future medical care and to have practitioners and staff provide care that is consistent with these directives. Advance directives allow you to provide directions on how you would like decisions to be made about your health care should you become physically or mentally unable to do so for yourself.
