Every patient has the right to complete a written statement about future medical care and to have practitioners and staff provide care that is consistent with these directives. Advance directives allow you to provide directions on how you would like decisions to be made about your health care should you become physically or mentally unable to do so for yourself.
Participating in Your Care – Speak Up
You, as the patient, can play a vital role in making your care safe by becoming an active, involved and informed member of your health care team.
We encourage you, as a patient, to speak openly with your health care team, take part in your treatment
choices, and promote your own safety by being well informed and involved in your care.
